PLEASE READ!
This is the website you can use to get your Medical record, Vaccination Record, make an appointment, and input History (Medical history, Family History, Allergies.)
In order to meet all the requirements for ACA (Obamacare), you MUST log onto YourHealthFile.com and set up your child's account. Please use Chrome or Firefox; Internet Explorer does NOT work, We will e-mail you a starting ID# and password that you will customize. You will then be able to manage your child's Health record.
To set up YourHealthFile:
Activate your portal with the Initial User name and password we sent you (use "Activate your account here" button in the upper left ORANGE box.)
Add or correct Demographics and Insurance information. ALL YELLOW boxes must be filled in.
From this point on, you will need to use the "Check In" button PRIOR to every appointment. The button is on the Home page or the Appointment tab.
If your online check-in is NOT completed within 15 minutes of your appointment, you may be rescheduled.
You need to answer ALL the questions over the next several pages which will populate the information in my record for the next visit. Make sure you hit “SAVE” and then “next” button, or your information will not be saved.
Before EACH visit, there will be a set of questions that you will need to answer, even if the appointment is a “Well” visit. If these questions are not answered before the appointment, we will probably have to reschedule for a later date.
